Amending the details included in the birth register
- In a birth registration note
- Column 01 Details (Date and Place of Birth)
- Column 03 Details (Gender)
- Details in column 05 (All maternal information) (When the maternal name is changed after birth registration, name change cannot be done under these provisions)
- The details in column 09 (informant information) can be corrected.
- Submit the declaration to the Additional District Registrar of the Divisional Secretariat in the area of birth.
- The declaration should be submitted,
- The person who is born
- Father or mother
- Legal Trustee
- Someone who is dissatisfied with some of the details in the birth certificate
- The fee for the declaration is Rs. 50.00.
- A certified copy of the birth certificate to be amended must be attached to the declaration. Provide written evidence to substantiate the claim.
- The following is some of the written evidence that can be used to substantiate the claim made in the declaration.
- Marriage certificate of the parents
- Child Directory / Sibling Directory
- Birth certificate of father / mother
- Birth of a child born after the parents' marriage is registered
- Birth of an older or younger child to the subject child when there is no child born after marriage registration
- Certified copies of records in hospitals, maternity homes or copies of midwifery documents
- Student Performance, Excerpt from the Children's Admission Document
- A copy of the ballot paper containing the subject and the names of the parents
- Death certificate of the father at the time of his death
